Two features change outcomes. Almost everything else is marketing.

Toothbrush marketing is unusually noisy for a category where the evidence is fairly settled. The things that measurably change gum health are how long you brush, how hard you press, and whether you clean between teeth at all. Bristle count, head shape and brand are largely noise around those three.
Which makes the electric-versus-manual question simpler than it looks. An electric brush is not inherently better at cleaning; it is better at making you brush for two minutes and stopping you scrubbing. A timer and a pressure sensor are the features doing the work, and a manual brush used carefully for two minutes beats an expensive electric one used for forty seconds.
The other thing worth knowing: most people brush too hard, and gum recession from over-brushing does not grow back. Soft bristles are what dentists recommend, and medium and hard sell better anyway — which tells you the market is not optimising for your gums.
You want the cheapest brush that actually helps
The Oral-B Pro 1000. It is the least expensive brush with both a real pressure sensor and a two-minute timer, and those are the two features that change outcomes rather than the ones that sell units. Everything above it in the range adds modes and app connectivity, which is not where the benefit is.
You want an electric brush for less
The Aquasonic Black Series. A sonic brush at a fraction of the big-brand price, and it does the job. The honest trade is replacement heads: less widely stocked, so check they are still available before you commit to a brush that needs them every three months.
You would rather use a manual brush
The Colgate Extra Clean in soft, and the word soft is doing the work. Two careful minutes with this beats a rushed thirty seconds with anything electric. Buying in a six-pack matters more than it sounds — a brush past three months is measurably worse, and people replace them late because they have to go and buy one.
Cold drinks hurt
Sensodyne Repair and Protect. The potassium nitrate works by calming the nerve rather than coating the tooth, which means it needs a couple of weeks of consistent use before you notice. It is not an on-demand painkiller, and people abandon it at day three deciding it did nothing.
You want to avoid fluoride
The Boka Ela Mint, which uses nano-hydroxyapatite instead. This is a deliberate choice with real trade-offs, not a straightforward upgrade — fluoride has decades of evidence behind it and nano-hydroxyapatite has less. Worth reading about before you switch, particularly for children.
You do not floss and know you should
The COSLUS water flosser is easier to keep doing than string floss, and something you actually do beats something you do not. But it complements floss rather than replacing it: water reaches around teeth well and between tight contacts less well. If you already floss properly, this adds little.
